Brief Electronic Paperwork
We have switched to paperless intake forms which are the most effective way of getting your information to our team of doctors and therapists. Please click on the following link to access our patient portal. Upon scheduling your initial appointment our staff should have emailed you instructions on how to proceed.
1) You will need a PIN provided by our office which will be emailed to you.
3) Click on “Patients”
4) Click on “Not registered? Click here for our secure registration”
5) Complete the 5 Steps.
6) It will send you an activation email. Click on the link included in the e-mail, or copy and paste it into your browser. This will take you back to the patient portal. Enter the username and password that you created.
7) When you login, you MUST complete these 4 sections:
-Required forms including Health History and New Case Intake
*We understand that this is a timely process and we recommend that you do this when you have adequate time to spend on it. Should you have any trouble filling out any or all of this online, please arrive 15-20 minutes BEFORE your scheduled appointment time to do so in our office.
Once the electronic paperwork has been completed, you will have a consultation with Accelerate Health to discuss your health-related problems, concerns, as well as potential treatment options.
History and Examination
In order to determine what your actual problem is, the doctors at Accelerate Health will ask you various questions related to your condition. Next, a number of specialized tests will be performed to determine which tissues are affected and to what extent.
Your specific condition may require x-rays to be taken to either rule out more serious conditions or assist in developing the most effective treatment plan for you.
Same Day Treatment
Once finished with the examination and all appropriate studies, patients will generally be provided with their first treatment during this same visit. This may include spinal adjustments, physical therapies, and/or soft tissue therapies.
Prior to leaving, patients will be given instructions on certain activities or procedures to be conducted at home or throughout the day. This may include ice or heat application instructions, avoidance of certain activities or positions, as well as exercises and/or stretches.
Schedule Your Next Appointment
You will be given a convenient appointment time for your follow-up visit. Generally speaking, patients are seen within 1-2 days and are provided with a complete review of findings. Treatment options for your condition will also be discussed during this time.
